COMMUNITY RESOURCE NAVIGATOR - LPN - Lawrence Coutny: Ohio LPN required  Must possess valid, clean drivers license. Coordinates all services to meet the person’s goals. Monitors overall service delivery. Coordinates treatment planning. Completes clinical charting in a timely manner.  Accepts overall responsibility for completeness and timely submission of assigned case load clinical records in accordance with ODMH/ODADAS standards. Meets established billable activity according to Agency requirements. Advocacy for, and linking to, community resources.  Serves as a client advocate and a liaison to other community agencies. Provides or arranges transportation service to appropriate appointments. Assesses strengths and weaknesses of client and family. Maintains confidentiality. Provides appropriate applications of crisis intervention. Leads or co-leads appropriate groups if necessary and beneficial. Remains current in general knowledge of psychotropic medication, side effects, and the DSM-IV diagnosis. Demonstrates punctuality and meets attendance guidelines. Demonstrates willingness to seek and accept supervision and assistance with job duties and problems. Maintains positive attitude, understanding and promoting Agency programs, mission, policies, procedures and standards. Other duties and responsibilities as assigned.

CARE COORDINATOR-Adams County: Applicant must have Ohio Licensed Social Worker or higher.  Minimum bachelor degree in related field.  Previous mental health work experience preferred.  Position will be responsible for overall care management and care coordination.  Position will also be responsible to provide comprehensive assessment, care plans, intakes and maintain productivity / outcomes targets.  Work in the community will be required.

CARE COORDINATOR / MENTAL HEALTH THERAPIST-Adams county: This position requires a minimum LISW through the state of Ohio. This position will in part be accountable for overall care management and care coordination, providing and coordinating all of the health home services and of the consumer's care plan which will include both medical/behavioral health and social service needs and goals. Position will also provide psychosocial psychotherapy for clients including individuals, families, groups and couples. Provides therapy based upon clinical assessment and diagnosis; development of treatment plans and engagement of the client in therapeutic contact to work on the client's identified problems.

Shawnee Mental Health Center, Inc. has been designated by the National Health Service Corp as being certified HPSA (Health Professional Shortage Area) sites for mental health services.  This means that certain licensed professionals are eligible to apply for the student loan repayment program.

Most clinical positions at Shawnee Mental Health Center require that the applicant be 21 years of age or older.  Also, most positions require that applicant possess an active drivers license with an acceptable driving record.
