JOB DESCRIPTION:

JOB SUMMARY

The Treatment Specialist works with people of all ages and their families/significant others to determine the goals for treatment and assists the client with meeting the identified goals.

 

Staff may be identified as a specialist in a specific program or service, such as children’s treatment specialist or addictions treatment specialist, whereas those not specified would be considered generalists.  If a specialty is designated then the staff person would be responsible for providing services to that population.

WORK SITE

The Treatment Specialist is expected to provide services in a variety of locations in order to meet the clients’ needs.  Therefore, the person may travel to client homes or other community locations to provide services as well as provide clinic based services. May travel to other SFHC locations based on needs of service.

 

DIRECT SERVICE REQUIREMENTS

People in this position are expected to primarily provide direct services at the top of their licensure and the majority of their work time should be dedicated to the provision of direct client care.  This position has required monthly targets for direct service, treatment planning and for health outcomes as designated by management.  The treatment specialist is expected to meet or exceed these monthly targets.

CORE DUTIES

    Conducts brief and comprehensive assessments.

·         May provide CPST and/or  Day Treatment Services, if assigned this duty by Supervisor.  Day treatment guidelines are:

o    Leads or Co-leads structured, group activities and curriculum to at least four or more persons up to four days per week for a minimum of three hours a day and meet billable productivity requirements in accordance with agency standards

o    Adheres to state / CARF standards for day treatment program and seeks and accepts supervision and assistance with job duties and problems

o    Contact clients within seven days of referral to the day treatment program and implements

o    Provides assistance and linkage for transportation to day treatment program

o    Transition and discharge plans for clients participating in day treatment must include a plan for program completion. Transition and discharge criteria are contingent upon the strengths, needs, abilities and preferences of the person served.

  • If hired as a specialist in a particular area, such as addictions or children’s specialist; may develop and conduct groups that focus on the specific needs of the population of focus.
  • Develops and updates individualized treatment plans for persons served per agency protocol.

      Educates the client and people within their support system (as needed and applicable)  about behavioral health diagnoses to help them develop a better understanding of their illness

·         Provide care coordination based on the needs of the person served

·         Provides individual psychotherapy

·         Provides family psychotherapy when applicable

·         Provides group psychotherapy 

·         Provides crisis psychotherapy and hospitalization screening services to clients and needed support to family

·         Supports the individual and family through the recovery process

·         Ensure implementation and coordination of the client’s  treatment plan

·         Identify gaps in overall treatment and assess what types of services the client/family need.

·         Provides community psychiatric supportive treatment based upon client needs and/or substance use disorder case management

·         Completes drug screens using the agency’s identified screening tool and implements the SBIRT protocol when a person screens positive.

·         Ensures that all required outcome data is documented as required by agency’s standards and timelines for all clients on caseload

·         Conducts suicide risk assessments using the Centers approved tool for clients identified at risk of suicide

·         Develops safety plans in collaboration with the client and family per agency guidelines

·         Ensure continuity of care during hospitalization, incarceration or admittance to residential facility with contact & follow up

·         Demonstrate computer proficiency in current EHR system.

·         Promotes overall wellness, recovery & resiliency.

·         Remains current in general knowledge of psychotropic medication, side effects & current DSM diagnoses.

·         Completes clinical documentation in a timely manner. And per agency guidelines

·         Maintains current knowledge of community resources available to consumers.

·       Attends staff meetings and adheres to work schedules, as needed for an effective daily operation.

·       Maintains good community relations and represents the Agency within the community.

·       Remains current in clinical training as appropriate.

·       Promotes the agency vision and mission and supports the agency’s values by exhibiting these values through their work and their everyday behavior and by recognizing colleagues for exhibiting the values.

·       Must be able to work flexible schedule as needed.  May be required to participate in after-hours, on-call program.

·       Other duties and responsibilities as assigned.  Demonstrates adaptability, flexibility, and responsiveness to needs of the Agency by accepting new and/or reassigned duties as requested.
